1. Purpose of This Notice
This Consent Notice explains the consent choices that may be presented in MatriArch Nexus™ and related pilot, beta, early-access, family care, care coordination, AI, dashboard, and integration features operated by MatriArch Technologies Inc. (“MatriArch,” “Company,” “we,” “us,” or “our”).
Consent settings are designed to give users control over health-related information, family records, AI-supported features, care-team sharing, connected-system workflows, pilot participation, and enterprise reporting boundaries. This notice should be read together with our Terms of Use and Privacy Policy.
2. Consent Choices at a Glance
Depending on your account type, pilot site, organization, permissions, and enabled features, you may be asked to consent to some or all of the following:
- Terms of Use and Privacy Policy acceptance
- Health data storage and account processing
- Family profiles and dependent care records
- Medication, supplement, allergy, appointment, and routine tracking
- Health journaling, symptom logging, and event timelines
- Baseline intelligence and trend observations
- AI-generated summaries, suggested tags, and care preparation notes
- Caregiver, family, provider, or care-team sharing
- Connected-system imports, exports, and interoperability workflows
- Pilot participation, onboarding, training, and support workflows
- Pilot dashboards, adoption metrics, and outcome reporting
- Feedback, surveys, interviews, testimonials, or case-study approval
Some consents are required to use the core service. Others are optional or may apply only to certain pilot, enterprise, family, AI, or integration features.
3. Required Consent: Terms, Privacy, and Core Service Use
Before using MatriArch Nexus™, you must accept the Terms of Use and acknowledge the Privacy Policy. These documents explain the rules of the service, the scope of permitted use, healthcare disclaimers, data practices, account responsibilities, and legal terms that apply to the platform.
If you use MatriArch Nexus™ through an organization, pilot sponsor, healthcare provider, employer, nonprofit partner, or enterprise customer, additional pilot terms, consent forms, privacy notices, Business Associate Agreements, data processing agreements, or written contracts may also apply.
4. Required Consent: Health Data Storage and Processing
The service cannot function unless it can securely store and process the information you choose to enter, upload, import, or create. This may include journal entries, symptoms, severity values, event timelines, baseline notes, medication schedules, medication check-ins, supplements, allergies, side effects, weight entries, appointments, care routines, tags, care notes, files, attachments, and family care records.
This consent allows MatriArch Nexus™ to host, store, display, organize, back up, secure, transmit, retrieve, and process your information as needed to provide the service, honor your settings, support your account, maintain audit records, troubleshoot issues, and comply with applicable legal or contractual obligations.
5. Health Data, PHI, and Consumer Health Data Context
Health-related information in MatriArch Nexus™ may be sensitive. Depending on the deployment context, it may be Protected Health Information (“PHI”) under HIPAA, consumer health data under state privacy laws, personal information, or other regulated data.
MatriArch Technologies Inc. is not automatically a HIPAA Covered Entity merely because users enter health-related information into the service. If we process PHI on behalf of a HIPAA-covered healthcare provider, health plan, healthcare clearinghouse, or Business Associate, the applicable Business Associate Agreement, healthcare organization’s Notice of Privacy Practices, HIPAA-required terms, and other written agreements may control where they conflict with this notice.
6. Optional Consent: AI Insights and Automated Support
You may be asked to allow MatriArch Nexus™ to process information using artificial intelligence or machine-assisted tools. AI-supported features may generate summaries, suggested tags, timeline organization, baseline comparisons, trend observations, pattern observations, care-preparation prompts, dashboard summaries, or other informational outputs.
AI outputs may be incomplete, inaccurate, outdated, biased, or inappropriate for your circumstances. They are not medical advice, diagnosis, treatment, medication instructions, emergency guidance, or a substitute for professional clinical judgment. You are responsible for reviewing AI outputs before relying on them or sharing them.
AI inputs, outputs, and related metadata may be stored, logged, reviewed, or evaluated to provide the feature, improve reliability, monitor safety, troubleshoot errors, maintain audit records, and comply with applicable obligations. Unless permitted by a specific consent flow, written agreement, applicable Business Associate Agreement, Privacy Policy, or law, we do not use identifiable health information or enterprise PHI to train generalized public-facing AI models.
7. Optional Consent: Family Access, Caregiver Roles, and Dependent Records
Family access features may allow you to create household profiles, manage records for a child or dependent, coordinate care for an elder or family member, invite trusted users, assign caregiver roles, or share selected information with authorized family members or support people.
If you enter, manage, import, or share information about another person, you represent that you have the appropriate permission, guardianship, caregiving authority, legal authorization, personal representative authority, or another valid basis to do so. You are responsible for respecting custody, guardianship, privacy, access, and consent rights that may apply to family or dependent records.
Changing or revoking family access may not remove information that an authorized recipient already viewed, downloaded, exported, copied, printed, or retained before the change took effect.
8. Optional Consent: Care-Team Sharing
Care-team sharing features may allow you to share selected information with healthcare providers, care coordinators, caregivers, family members, community partners, case managers, support staff, or other authorized recipients. Sharing should be user-directed, scoped, permission-based, and logged when supported by the service.
Shared information may include selected journal entries, symptom summaries, medication context, allergy information, appointment notes, care routines, care preparation notes, family records, or AI-generated summaries. You should review all information before sharing it and should not use MatriArch Nexus™ as the only source of truth for medical decisions.
9. Optional Consent: Connected Systems, Imports, Exports, and Integrations
MatriArch Nexus™ may support optional integrations with healthcare systems, EHR platforms, patient portals, health information exchanges, interoperability networks, labs, pharmacies, scheduling systems, identity providers, APIs, cloud services, analytics tools, document storage providers, or other third-party systems.
If enabled, connected-system workflows may allow information to be imported, exported, matched, requested, transmitted, displayed, updated, or shared. Third-party systems are governed by their own privacy policies, security practices, contractual terms, and legal obligations. Once information is sent to a third party at your direction or under an authorized integration, that third party may control how it uses, maintains, corrects, deletes, or discloses the information.
Integrated data may be incomplete, delayed, duplicated, unavailable, incorrectly formatted, or inconsistent with the source system. You should verify important information with the appropriate healthcare professional, organization, caregiver, or source system before relying on it.
10. Pilot, Beta, and Early-Access Participation
If you participate in a pilot, beta, early-access, limited-release, healthcare-sponsored, employer-sponsored, nonprofit-sponsored, or enterprise-sponsored program, you may be asked to consent to pilot-specific workflows. These may include onboarding, training, support, surveys, interviews, feedback, feature testing, workflow validation, dashboard access, adoption tracking, outcome measurement, and customer success reporting.
Pilot features may be incomplete, experimental, unavailable, modified, suspended, or discontinued. Participation does not guarantee continued access, commercial availability, specific functionality, clinical accuracy, regulatory approval, future pricing, or continued pilot eligibility.
Pilot reporting boundary: Pilot sponsors may receive administrative dashboards and reports showing activation, onboarding progress, training completion, support volume, feature usage, workflow completion, satisfaction trends, adoption metrics, estimated time savings, ROI indicators, and aggregated outcome indicators. Unless you direct us, applicable law allows it, or a governing agreement permits it, we do not disclose your personal journal entries, symptom details, medication details, family records, or other identifiable health content to a pilot sponsor solely for general adoption reporting.
11. Feedback, Surveys, Interviews, and Case Studies
You may choose to provide product feedback, feature requests, bug reports, survey responses, interviews, workflow comments, testimonials, or pilot success feedback. We may use feedback to improve, develop, validate, commercialize, and support MatriArch Nexus™.
We will not publicly identify you, disclose identifiable health information, or use your name, image, organization, story, testimonial, or case-study details in public marketing materials without appropriate authorization or written approval. De-identified or aggregated pilot results may be used for product improvement, operational analysis, investor or enterprise diligence, benchmarking, research, and business purposes as described in the Privacy Policy and applicable agreements.
12. Communications, Training, and Support Consent
We may send administrative, security, support, onboarding, training, pilot, policy, and service-related communications. These messages may be necessary to operate your account, support your pilot, notify you of material changes, maintain security, or provide training and customer success support.
You may opt out of non-essential marketing communications where available, but you may still receive important service, legal, security, pilot, or account messages.
13. Service Providers and Processing on Our Behalf
MatriArch Nexus™ may rely on service providers that support hosting, cloud infrastructure, authentication, security, analytics, support, communications, AI processing, payment, legal, compliance, and operational services. These providers may process information on our behalf under contractual, security, and privacy controls.
Where required by law, a Business Associate Agreement, data processing agreement, enterprise agreement, or other written contract may apply to service provider processing.
14. Consent Records and Audit Logs
When you make, update, or revoke a consent choice, MatriArch Nexus™ may store a consent record. Consent records may include your user ID, account identifier, organization or pilot context, consent status, consent scope, policy version, timestamp, revocation status, authorization records, access grants, share events, IP-related information, device or session metadata, and audit-oriented activity logs.
These records support auditability, security, compliance, troubleshooting, user trust, pilot administration, access control, and enforcement of your choices.
15. Revoking or Changing Consent
You may be able to update or revoke certain consent choices through product settings or by contacting us. Revocation may limit or disable features that depend on the consent, such as AI insights, care-team sharing, family access, connected-system workflows, data import/export, or pilot dashboard participation.
Revocation does not necessarily delete information that was already lawfully processed, retained for legal or contractual reasons, maintained in audit logs, stored in backups, de-identified or aggregated, or shared with an authorized recipient before the revocation took effect. Deletion, export, and correction requests may be subject to the Privacy Policy, applicable law, Business Associate Agreements, healthcare organization requirements, and technical feasibility.
16. No Medical Advice, No Provider Relationship, and No Emergency Use
MatriArch Nexus™ is for informational, organizational, educational, and care coordination purposes only. It does not provide medical advice, diagnosis, treatment, emergency care, clinical monitoring, or professional healthcare services. Use of the service does not create a physician-patient, nurse-patient, therapist-patient, pharmacist-patient, care manager-patient, or other healthcare provider relationship between you and MatriArch Technologies Inc.
The service is not an emergency response system and is not monitored for urgent medical communications. Do not use MatriArch Nexus™ to report emergencies, urgent symptoms, adverse drug reactions, abuse, self-harm risk, or time-sensitive medical conditions. If you believe you or another person may have a medical emergency, call emergency services immediately.
17. Children, Dependents, and Guardianship
MatriArch Nexus™ is intended for use by adults and authorized guardians, caregivers, personal representatives, or legally authorized users. The service is not directed to children for independent use. If information about a child, dependent, elder, family member, or other individual is entered, imported, managed, or shared, the user doing so represents that they have authority to provide and manage that information.
If we learn that a child has created an account without appropriate authorization, we may delete or restrict the account and related information as required by law and operationally feasible.
18. Changes to This Consent Notice
We may update this Consent Notice as our service, pilot programs, integrations, AI features, legal obligations, or business operations change. If changes are material, we may provide notice, update the “Last Updated” date, or require you to review and accept updated consent terms before continuing to use certain features.
